Companion Animals…
The Jake Brady Memorial Fund, a 501(c)(3) Non-Profit Organization, is established
in the life and memory of Jake Brady, an extraordinary Dalmatian
who shared his love for 14 years and 23 days. Sadly, Jake
passed away on February 17, 2006 from a chronic kidney disorder,
one of the three major silent diseases affecting adult Dalmatians.
He is truly missed, and will live forever in our hearts.
FUND OBJECTIVE
The objective of the Jake Brady
Memorial Fund is simple: To
Provide Financial Support to companion animals in need for the prevention and treatment of major health ailments including, but not limited to:
- Kidney Disease and Renal Disorders
- Seizures and Epilepsy
- Blindness
GOAL
Our goal is to provide for quality care with a focus on major occurrences (i.e. not
for routine veterinarian visits) which may include:
- Emergency Treatment
- Surgical Procedures
- Testing, Medications, and Vaccinations
- X-rays
- Preventive Therapy
- Room and Board
FUNDING ASSISTANCE
Fund recipients of companion animals include:
- Senior citizens and retirees on fixed income
- Low income families
- Physically disabled
- Unemployed and/or receiving government assistance

Memorial Fund guidelines, and to receive a request for funding application,

message center where we will receive your request and respond as soon as possible.
The Fund services the continental United States with an emphasis on the
State of Ohio, taking all qualified applications under consideration. Determination is
made through a formalized funding application process reviewed by our Board of Trustees,
and is based on the financial situation of the family involved, whether the companion
animal will lead a quality, healthy life after specialized medical procedures are
completed, and the amount of funds the Jake Brady Memorial Fund has available at time
of application.

Applicants may utilize a veterinarian facility of their choice
within the continental United States.
* An annual accounting of donations and disbursements will
be compiled annually to each donor and recipient, and a Jake Brady
Memorial Fund newsletter will be distributed twice per year.
